    PLEASE READ BEFORE SIGNING If you have any questions regarding this statement, please ask our employment interviewer before signing. It is my understanding that Monsignor Bojowski Manor will make a thorough investigation which will include a verification of my entire work history, and the verification of all data given in this application. I authorize such an investigation and the giving and receiving of any information by the Manor. I am aware that I have a right to make a written request as to the nature and scope of this investigation. I release from liability any person giving or receiving such information. In the event of my employment by Monsignor Bojowski Manor, I will comply with all rules and regulations as set forth in the Manor’s policy manual or the communication distributed by the Manor to all employees. I understand that if I am hired, my employment with the Manor will be "at will" and may be terminated by the Manor or me at any time for any reason or for no reason. I understand that no documents or statements of the Manor will constitute a contract or employment that in any way limits the Manor’s rights to terminate my employment “at will.” I further understand that the “at will” nature of my employment cannot be changed except by a formal written contract signed by the Administrator of the Manor.   • PRE-EMPLOYMENT PHYSICAL EXAMINATION & DRUG TESTING RELEASE FORM

    In order to ensure the safety and well being of all our employees, we have devised a physical examination that helps to ensure that all potential employees are physically capable of handling the type of work that he/she may be required to perform. Included in this physical examination are tests for substance.

Further, I release Monsignor Bojowski Manor, its officers, directors, employees and agents, in both their individual and representative capacities, from any liability in connection with the physical examination and related tests.
